-Leagues start immediately following Summer sessions. -The Leagues will be 6 weeks long followed by a single elimination tournament. -A $75.00 league fee will be collected with the submission of this signup sheet. -(CASH or CHECK only)- Checks made payable to Doug Tackett -All signups must be registered by Monday, August 29th. Complete rosters must be turned in at first match. -Each day is a separate League. Teams may play in multiple Leagues. (i.e.-Tues C & Thurs C are two separate leagues and may both be played by the same team as long as fees are received for both) -Each League has a capped amount of teams, so please return signup and fee ASAP -All League play (Mon-Thurs) is Coed, 5v5. Two women must be on the court at all times for each team. -League play on Fridays is Coed Quads. One woman must be on the court at all times for each team. -Level of play is divided as follows: B or above-highest talent pool offered. Club volleyball talent level or better C-good players with volleyball knowledge and above average skill. D-players learning the game or just don’t possess the skill level to compete with higher levels -Please try to pick the appropriate level to ensure a fun, competitive league for all teams involved. DON’T SANDBAG IN LOWER LEVELS OR SUBJECT YOUR TEAM TO EMBARASSMENT IN THE HIGHER LEVELS!!!

MARIKKA’S VOLLEYBALL LEAGUES ROSTER and WAIVER / RELEASE LIABILITY
In consideration of being allowed to participate in any way in the Marikka’s volleyball leagues the undersigned:
Agree that prior to participating, they will each inspect the facilities and equipment used and if they believe anything is unsafe, they will refuse to participate.
Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions , but the inaction’s or negligence of others, the rules of play, the condition of the premises or of any equipment used. Further, that there may be other risks unknown to us or not reasonably foreseeable at this time.
Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death.
Release, waive, discharge and covenant not to sue Marikka’s Inc. and any affiliated clubs, their respective administrators, directors, agents, and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lease’s of the premises used to conduct the event, all of which are hereinafter referred to as “releases, from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damage on account of injury, including death or damage to property, caused or alleged to be caused in whole or part by the negligence of the release or otherwise.
THE UNDERSIGNED HAVE READ THE ABOVE WAIVER AND RELEASE , UNDERSTAND THAT THEY HAVE GIVEN UP SUBSTANCIAL RIGHTS BY SIGNING IT AND SIGN IT VOLUNTARILY.
-All of the roster members must be 21 with valid I.D. -Players may not be on 2 rosters of the SAME league. -Any team or player may be ejected from the league, without refund, due to bad conduct or trying to use players under 21.
